Nasya
The nose is the doorway to the brain, mind and consciousness. Prana, or life-force energy, enters our bodies through the breath so it is vital that the nostrils are free from blockages, well lubricated and in a healthy condition. The Nasya treatment aids the lubrication and cleansing of the nostrils and stimulates other benefits throughout the whole of the neck and head.Nasal is the nasal administration of medicinal herbs and oils for the purpose for treating a whole range of ailments from the neck up. Dry nasal passage, sore throat, hoarse voice, stiffness in the neck of head, jaw ache, facial paralysis, headaches, migraines and various eye and ears problems. It also relieves mental and emotional stress, anxiety, fear and negativity. Different oils are used to treat differing symptoms or aliments. Nasya oil is infused with special herbs that both nourish and lubricate the sinus cavities.

Pizhichil
Pizhichil is a special form of Ayurvedic treatment that combines rich oil massage with a special heat treatment. This therapy is known as the “King’s Treatment” due to its popularity with the Maharajas of ancient India.
During this indulgent treatment 20 liters of medicated and warmed Ayurvedic oil is continuously poured over the body while two practitioners rub and stimulate the skin using hands and special cotton cloths. The process is performed with the person both lying down and while sitting in a chair specially designed for the purpose of applying medicated oils.This is a highly rejuvenating treatment that enhances muscular tone and re-tunes the entire nervous system. It comes as a package in itself – a course of 10 days is necessary to gain its maximum healing potential.

Netra Tarpana
Netra Tarpana is a special Ayurvedic treatment for the eyes. It relieves tiredness and also improves eyesight. It is highly recommended for people who regularly work at computers, operate machinery, drive for long periods or anyone who is currently suffering from tired, aching and sore eyes.
This Ayurveda treatment for eyes is carried out in combination with a face massage and is experienced by our guests as being very enjoyable, relaxing and effective. This therapy takes around 30 minutes in total.
Freshly made dough rings filled with fragrant oils are placed around the eyes, sterilized cows ghee (a clear butter-like liquid) is gently poured onto the eyes while they are kept open. It is very effective at treating eye diseases and poor or blurred vision.

Abhyanga
Abhyanga is an integral part of the daily routine recommended by Ayurvedic doctors in order to promote overall health and well-being. The prefix “Abhi” means “into or towards” and “ang” has a root meaning of “movement’. So “Abhyanga” means to move energy into the body whilst moving toxins towards the body’s eliminatory systems.
This is a gentle but firm warm oil massage during which 2 practitioners work in synchronized harmony to activate and gently awake the entire body. Special herb-infused Ayurvedic oils are slowly warmed up and gently massaged into the skin. Oils are sterile before use and are chosen according to the clients’ prakruti (psychosomatic constitution or dosha) .
The massage is deeply relaxing and harmonizes, detoxifies, strengthens and revitalizes the entire body. The massage also improves lymph flow and stimulates the immune system. Furthermore, whilst providing a tired person with new energy, it simultaneously calms and strengthens the nervous system. After the procedure the person is directed to take a herbal steam bath followed by a hot shower.

Abhyangam is also an important and integral part of Panchakarma procedure as pre-operative procedure. Here the oil is applied to the entire body with a particular type of massage which helps the toxins to move towards the gastrointestinal tract. This also makes the superficial and deep tissues soft and supple.
Shirodhara
A relaxing therapy where warm medicated oils are poured from a special vessel onto the center of the forehead, inducing a mental state of deep relaxation. It calms the nervous system and helps to melt away the chattering of the mind.
In Sanskrit language Shirodhara is composed of two words shiro + dhara. Shiro means head and Dhara means flow of warm oil. The patient lies supine on a flat surface with head back and a rolled towel or pillow beneath the neck. A stream of warm oil is drizzled very slowly onto the center of the forehead (the location of the “third eye”) for 60 minutes followed by gentle scalp massage. The effect is indescribable.

Kati Basti
Kativasthi
Kativasti is a special type of low back therapy or lumbar therapy. The word Kati stands for low back or waist and Vasti ,for containing or retaining something inside. Kativasthi is an Ayurvedic lumbo-sacral rejuvenation therapy using hot oil or herbal decoctions. One can achieve both local oleation and sudation by this procedure.
PROCEDURE
- A dough is prepared out of black gram flour or whole wheat flour and water.
- Medicated oil or herbs for decoction is selected depending on the bodily constitution and the state of the humors (Doshas) of the person.
- The person is made to lie face down on the Massage Table (Droni).
- The dough is made into a ring of four to five inches in diameter which is placed over the lumbo-sacral area of spine.
- After it is glued with some water to make it leakproof, lukewarm medicated oil or herbal decoction is slowly poured into it. When this cools down it is squeezed out and replaced with more warm one.
- At the end of the procedure the dough is removed; a gentle massage is given over the area. The person is made to take rest for a while.

Greeva Basti
Greeva Basti
The phrase Greeva Basti is from Sanskrit. The word Greeva refers to neck and Basti stands for container or retaining something inside. It is an Ayurvedic neck care and basically a hot oil therapy.Greeva Basti is bathing the neck region with medicated oil or freshly prepared herbal decoction. The procedure does oleation and sudation around the neck region simultaneously. Due to present day lifestyle which involves continued and glued sitting position and lack of body movements the problems of the spine, muscles and skeletal tissues of neck and shoulder arise. Greeva Basti addresses these issues effectively.
PROCEDURE
- The person undergoing Greeva Basti is made to lie face down on the massage table (Droni)
- The dough is prepared out of black gram flour or whole wheat flour. It is then made into a small ring of four to five inches in diameter which is placed over the neck to cover all the vertebrae of the neck and 2-3 vertebrae of the thoracic region.

Janu Basti
Janu Basti
The knee joint is called Janu Sandhi in Sanskrit; hence the name Janu Basti is given to the proceduredone on knee joints. In this the knee joint is bathed with warm medicated oil or freshly prepared herbal decoction. Depending on the condition it can be practiced over both the knee joints or on one knee joint. Knee joints are considered as sandhi Marma in Ayurveda (joint type of vital areas) which measure about three fingers (three anguli pramana).So the janu vasti is specialized knee therapy or massage which rejuvenate the janu marma.
TREATMENT METHOD
- The person is made to lie on his back on the massage table (Droni).
- Reservoir made of black gram dough is built upon the Knee Joint. The dough ring should be such that it covers the knee joint.
- After ensuring the ring is leakproof, the lukewarm medicated oil or herbal decoction is slowly poured into it. When that cools down, it is squeezed out with cotton gauze and the procedure is repeated.
- At the end of the procedure, the ring is removed and the affected area may be gently massaged The person is then made to take rest for a short while.

Pathrapinda Sweda (Elakkizhi)
This is an innovative classical massage with medicated leaves and is used to tackle joint pain, muscle cramps, stress and arthritis.
All the above-mentioned treatments can be done in a perfectly healthy person as well so as to enhance his/her immunity, vitality and longevity of life. Choorna Pinda Sweda
Pindasveda is a highly effective rejuvenation treatment designed to revitalize the entire body. A special type of rice that is cooked, tied into a cotton ball, dipped into a herbal mixture with warmed milk and then skillfully massaged into the body by two therapists simultaneously. Udvartanam
Udwarthanam (Dry massage)
Udwarthanam is special therapeutic deep tissue massage using herbal powders. Udwarthanam means to elevate or to promote. This massage involves upward stroking, where the powder is rubbed forcibly on the body. Classical treatises on Ayurveda explain this procedure as a daily routine.
Procedure:
The herbal powder is warmed by heating. The massage is done in the opposite direction to hair follicles by two therapists. The procedure is performed in seven postures. After completion the person is advised to take rest for 30 minutes, followed by a shower with warm water.

Udwarthanam Kaphaharam Medasaha Pravilayanam I
Sthireekaranam Anganam Tvakprasadakaram Param II
Meaning: Udwartana pacifies kapha dosha; liquefies the fat; stabilizes the body parts; best to improve the complexion of the skin
Navarakkizhi
Involves fomentation of the body with a bolus of cooked rice. Shastika or particular kind of rice known for its nutritional value, is cooked in milk and a herbal decoction made of bala(Sida rhombifolia) . Four boluses are made of this cooked rice and tied up in linen bags. The fomentation begins with the patients sitting upright on the droni with legs outstretched . The masseurs on either side perform the fomentation process in an identical manner. The movement of the hands during both fomentation and massage should always be downwards.The fomentation continues with the patient on his/her back on the droni. There is also another attendant who supplies new kizhis, which are kept ready in a decoction of warm milk. It is very important to keep the kizhis at a constant temperature. The treatment is over once fomentation has been carried out in all the seven standard position
Benefits: In some cases, one of the three supine position may be avoided and the patient is made to lie face down. After the specified duration of treatment, the paste is wiped off with palm leaf scrapers, a suitable type of oil is applied all over the body before the bath.
Uro Basti
Hrid Basti( Uro basti) in Sanskrit refers to Hridaya, the heart and Basti, the container or retainer. This is also called as Urobasti (Uro=chest and basti=retaining something inside). So Hrid Basti is an oil bath on the heart. This suggests an Ayurvedic hot oil treatment where the medicated hot oil preparation or freshly prepared herbal decoction is poured over the heart region. Hridaya, the heart is considered as one among the trimarma (three vital areas) measuring about four fingers (four anguli pramana), covering heart, middle of sternum.
This procedure is bathing the heart region using warm medicated oils or herbal decoctions or medicated ghee depending on the condition.

- The dough is prepared out of black gram flour or whole wheat flour. It is then made into a small ring of four to five inches in diameter which is placed over the chest to cover the heart i.e., to the left side of the sternum.
- After it is glued with some water to make it leakproof, lukewarm medicated oil or the herbal decoction is slowly poured into it. When this cools down it is squeezed out and replaced with more warm oil or herbal decoction.
- At the end of the procedure the dough is removed; a gentle massage is given over the area. The person is made to take rest for a while.

Shirovasthi
Shirovasthi is the procedure where in the medicated oil is retained on the head for a prescribed period. Two therapists perform this procedure.

Bashpa Sweda
The treatments found in Ayurveda date back thousands of years and Swedana is no exception. In this treatment you sit for 30 minutes in a herb-infused steam box. The use of heat and steam to encourage sweating has long been recognized as a natural and effective way to encourage the process of detoxification.
The special herbs help to stimulate oxygen flow throughout the body, while the steam activates the natural sweating process, which is essential for releasing accumulated toxins. Your skin will carry the gentle sweet aroma of the herbs throughout the following 24 hours or so.This process helps to restore unbalanced dosha back to their natural state.
Recommended for: Vata and Kapha dosha types in particular
Benefits: softens and brightens the skin, stimulates and increases digestion capacity, reduces laziness and fatigue, helps to ease stiff joints, helps to bring excess Vata dosha under control.
Valuka Sweda
Application of the heat and there by inducing perspiration by using heated pack of sand is known as Valuka Sweda. The procedure somewhat simulates shashtikashali panda sveda. Here in Valuka Sweda body part is subjected to sudation procedure without prior application of oil, and is the unique feature of this treatment. For the same reason in diseased conditions where snehana is contraindicated, svedana is best done by this procedure.
Ksheera Dhara
When this treatment is done with milk (Ksheera), it is called Ksheeradhara. It is performed in seven standard massage positions in Kerala and lasts approximately 40 minutes. Decoctions made of various herbs will be added to the milk. Ksheeradhara is of two types – shirodhara and sarvangadhara. While shirodhara is carried out for the head, Sarvangadhara covers the whole body except head. Ksheeradhara is effective in the treatment for hypertension, depression, anxiety, insomnia etc. Dhara with medicated milk is called Ksheeradhara.
Decoctions of various herbs are added to the milk. Ksheeradhara my be either for the head alone –Shirodhara. Or for the whole body except the head- Sarvangadhara. A Sravanga Ksheeradhra is usually prescribed for diseases caused by disorders of the Pitta . As a first step, a suitable type of oil is applied on the head. At least three attendants, two to pour the fluid and another to refill the vessels are required. The treatment is conducted while the patient goes through the standard seven positions. A nozzled pitcher is ideal for pouring the milk.
Podikizhi
Podikizhi
This is a type of Swedana Chikitsa (fomentation) wherein medicated powders are made into poultice and used for giving fomentation.
Duration of treatment: 60 minutes

Shiroabhyanga
Shiroabhyanga is a beautiful combination of the Shirodhara and Abhyanga treatments in one. Firstly, the deeply relaxing Shirodhara treatment is given; warmed medicated oils are gentle drizzled onto the forehead over 45 minutes. This delightful sensation melts away all chatter and mental activity leaving the mind calmed, cooled and refreshed. Secondly, the gentle but firm hand massage movements of Abhyanga are given to the entire head, neck and shoulders. This loosens tight muscles and encourages much needed blood circulation that flooding the head with oxygen-rich blood and helping to remove harmful toxins.
This treatment nourishes the five senses by focusing on the ‘marma points’, many of which are located around the head. These points, when gently massaged have a hugely calming and balancing effect throughout the body and nervous system. The oils used for this treatment are specifically chosen according to your dominant dosha constitution type.

Dhanyamla Dhara
Karnapoorana
Karana purana is the process in which warm, medicated oil or medicated liquid is gently, slowly poured into the ears. This lubricates the delicate filaments of the ear canal which sharpens hearing and removes impurities.
The treatment begins with an ear and partial head massage. The ear is then gently filled with drops of warm medicated oil.

- Karana Purana is the process in which warm oil or medicated liquid are poured into the ears. Bathing the ears in warm oil is calming and protective to the nervous system.
- Karnapoorana strengthens structures of ear specially external ear, tympanic membrane and middle ear.

Netra Prakshalanam
Ksheeradhoomam
This therapy begins with the application of suitable medicated oil on the face, neck and chest. The face and neck are then subjected to exude profusely through steam generated from a combination of cow’s milk and herbal decoctions. This therapy is effective for facial paralysis, trigeminal neuralgia, pimples and other skin problems.
Jaloukavacharana (Leech Application)
Jaloukavacharana (Leech application) the process is basically meant for purification of blood and to treat blood related disorders. The use of Leech in the treatment of many skin diseases is now known and accepted all over the world . Leech therapy is used in various skin disorders, like Eczema, Vitiligo, Psoriasis, Non –heeling ulcers, Varicose veins, Pigment disorders, Hematomas, Piles and other vascular disorders (Deep vein thrombosis).Various scientific studies have been established the efficacy of leech therapy.
A special treatment in ayurveda, Agni Karma is an application of heat to a diseased part with the help of a metal rod. In this treatment, the spots are selected according to the ayurvedic acupuncture points or viddha Chikitsa.

What is Agnikarma?
According to Ayurveda, Agnikarm is a superior para surgical procedure. This procedure aims at management of various afflictions by inflicting burns on the tissue surface directly by using different materials known as ‘Dahanopakaranas’(tools of cauterization).
What are tools of cauterization?
These are different materials subjected to heat and used for therapy in different conditions. The specificity of tools of cauterization depends on the disease status concerned. For superficial conditions, materials used are of minimum latent heat, whereas in deeper tissues used objects are with maximum latent heat. On this concept, in classics, for the diseases of skin surfaces the materials used for Agnikarma are Pippali (Piper longum), Aja Shakrit (feces of Goat), Godanta (Teeth of Cow), Shara (Arrow), Shalaka (Probes), Varti(Wicks), Suryakanta (Variety of stone) etc. Likewise, for the diseases of muscles Loha (Iron), Swarna (Gold), Tamra (Copper) and Kamsya (bronze) are used. In deeper and strong tissues like bones, ligaments, tendons, vessels etc., Madhu (Honey), Guda (Jaggery), Vasa(Fat), Ghrita(Ghee), Taila(Oil), Madhucchishta(Beewax ) are used.

How Agnikarma(cauterization) helps to relieve the pain?
Here the heat, which is transferred to skin acts by removing the obstruction in the Srotas(minute channela) and increases the blood circulation to the affected site. The rate of any metabolic activity is increased by a rise in temperature. From the therapeutic point of view with an appropriate rise in temperature, all cell activity increases, including dilatation of vessels, cell motility, synthesis and release of chemical mediators. More blood circulation, flushes away the inflammation and patient gets relief from the symptoms. Afferent nerves stimulated by heat is having an analgesic effect by acting on the gate control mechanism.
Is there any contraindication for Agnikarma (cautery)?
Persons of Pitta prakruti, emaciated, children, old aged, sensitive, pregnant, multiple ulcers in the body, anemic, obesed, indigestion, alcoholic, poison affected, low immunity etc are not recommended for this therapy.
Is there any complications?
Improperly performed Agnikarma will lead to severe burning sensation, increased pain, destruction of tissue, suppuration, bleeding, non healing ulcer formation etc.
Properly performed Agnikarma by skilled surgeon will give excellent result in several conditions. On the other hand improperly performed will lead to many complications.
Others
Shirolepa (Thalapothichil)
Effective in healing problems related to the head and neck regions like sleeplessness, Alzheimer’s disease, migraine, certain psychosomatic diseases, headache, mental illness, hypertension, sinusitis etc.
Shiropichu
A simple procedure of applying the oil to the vertex using a cotton pad impregnated with oil, medicated or otherwise is known as shiropichu. Once again here the application of oil does not follow any form of massage. This procedure is said to be more beneficial in dermatological conditions of scalp as well as psychological disorders. As this process does not involve massage, It can be confidently performed in painful conditions or ulcerations of the scalp.
Ishtika Sweda
Is a type of sudation where the brick is pre-heated (red hot) and water kanji is poured over it to release fumes and affected area is exposed to these fumes.
Avagaha sweda
Tub bath is swedana / sudating the patient by way of immersing the whole body in a tub which is filled with oil or herbal decoctions or milk (ksheera) according to the patients’ prakruthi (constitution).
Upanaha Sweda
This therapy is well-known for its property to relieve pain, swelling and to nourish the tissues. It is done by application of herbal paste over the affected area, covering the applied paste with medicinal herbal leaves and bandaging over that with cotton cloth. This application will be removed after few hours as per the requirement. This therapy is very effective in osteo-arthritis, joint injuries and localized pain and swelling.
Annalepa
Inducing perspiration by applying the warm paste of rice is referred as annalepa. The whole body below the neck or a portion of the body may be conveniently subjected to the svedana therapy by this procedure. In this procedure the rice that grows in 60 days is preferred for carrying out the treatment. This is an easy and effective procedure of swedana and is ideal in healthy persons, in children and also in clients suffering from illness due to morbid vata dosha.
This procedure is indicated both in healthy as well as diseased. More specifically this procedure is ideal in following disorders:-Hemiperesis due to different causes, Motor neuron disease, Bulbar palsy, Parkinsonian syndrome, Emaciation, General debility, Rheumatoid arthritis, Peripheral vascular disease.
Mukhalepa
The topical application of the herbal paste to the face and keeping it undisturbed for certain period is known as Mukhalepa. This therapy is used to clear the morbidity of dosha or any toxic substances in facial skin, as well as to improve the color and complexion of the skin. Accordingly there are three types of mukhalepa and are named as Doshaghna Mukhalepa, Vishaghna Mukhalepa and Varnakara Mukhalepa.
Application of warm herbal paste is preferred in clients suffering from morbidity of vata as well as kapha in the facial skin. Contrary to this when morbidity of pitta dosha is identified in the facial skin, the cold herbal paste is applied. So also, vishaghana and varnakara mukhalepa is also carried out by applying the cold herbal paste.
Anjanam
Application of Herbal extract with madhu or gulabijala inside the eyes helps to remove foreign matter from the eyes.
Dhoopanam
Exposure to medicated smoke originated from different drugs relevant to the ailments, especially in psychosomatic diseases etc.It is also done on different parts of the skin for certain diseases.
Gandoosha
Taking little medicated oil and swirling it between the teeth helps in strengthening the teeth and gums.
Marma Massage
This art is used to activate the pressure points in the body called “Marma”s for correcting the body’s energy flows and replenishing its resources. Marma massage is soft flowing massage can be done by two people massaging in synchronize rhythm or with single Individual.Marma massage the techniques and the guidelines are developed keeping the knowledge of channels and nadi location in priority.
The massage is 60 to 90 minutes of duration followed by steam bath or warm shower. Marma massage stimulates generates the own energy resources and gives person more vital energy . Snehapaana
In Snehapana, a prerequisite in Panchakarma, Tikta Ghruta or ghee infused with herbs is orally administered to the patient for a specific period. This ghee is made from clarified butter (obtained from cows milk) from which the protein content has been removed. It is then boiled with various herbs. Snehapana is done in the morning after attending to the calls of nature, on an empty stomach. The health, age, Dosha condition and digestive capacity of the patient need to be taken into consideration when determining the dosage. The treatment lasts between five and seven days depending on the condition of the patient and the illness being treated.
Upanaha sweda
Upanahasweda or poultice is a purvakarma in Panchakarma therapy in Ayurveda. It is the local application of medicated paste to cure pain and swelling. In this treatment herbal medical mixture is applied hot over the affected part of the body and is allowed to remain there for over twelve hours.
Avagaha sweda
The patient is made to sweat while sitting in a tub filled with a warm medicinal decoction prepared using suitable herbs for each health problem. According to Ayurveda, this kind of sweda is good for rheumatism, painful urination, hernia etc.
Vamana

Anuvasana Basti (Oil Enema)
Oil enemas are a central part of Ayurveda, in fact there are over 100 specific oil enemas listed in the ancient medical texts. Specially selected and prepared oils are gentle inserted into the rectum and allowed to absorb over time. Ideally the oil should be kept in for several hours, preferably over night.The oils find their way through the colon and absorb all sorts of oxidants and toxins along the way.
Basti is especially good for excessive Vata imbalances. Far more gentle than colonic irrigation, oil enemas offer an effective and gentle alternative way to cleanse and detox the colon. Since Vata is mainly located in the colon and bones, the medication is administered rectally. There are about 80 Vata related disorders in Ayurveda. About 80 percent of them can be treated with medicated enemas.

Hemorrhoids or Piles as is generally known is a very common disease. About 70% of the population are at some time in their lives suffered from piles or have had experienced some symptoms related to it.
We will first try to understand what Piles or Hemorrhoids. Piles or Hemorrhoids are actually, distended veins in the anal canal. Veins tend to stretch under pressure. When veins in the lower part of the rectum or anus stretch, swell or descend in to the anal canal, then they prolapse or protrude in to the canal. They are considered as Piles or Hemorrhoids.
The set of veins inside the rectum when distended result in internal piles and the set of veins under the skin around the anus when distended result in External Piles.

Causes
As described earlier, hemorrhoids develop because of increasing pressure in the veins of the lower part of the rectum and anus. This increasing pressure can be due to various reasons like

> Sitting or standing for a long time
> Chronic constipation accompanied with straining to evacuate the bowels
> Repeated diarrhea and dysentery
> A habit of suppressing the urge to defecate
> There may also be an inherited tendency to develop Piles.
Types of Hemorrhoids/ Piles:
Hemorrhoids / Piles can be classified in two ways
I. As per their site of development
- Internal hemorrhoids/ piles.
- External hemorrhoids/ piles.
II. As per the stages of the diseases
- First degree hemorrhoids.
- Second degree hemorrhoids.
- Third degree hemorrhoids.
- Fourth degree hemorrhoids.
First degree Hemorrhoids: Is the condition when the mass (The structure with the veins) projects slightly into the anal canal, but only when the veins are congested i.e. during defecation. In this stage the patient will have no mass protruding from the anus.
Second degree Hemorrhoids: The mass protrudes out of the anus, but only during defecation. The hemorrhoidal mass returns into the anal canal spontaneously after passing stool as soon as the accompanied straining stops.
Third degree hemorrhoids: The hemorrhoidal mass protrudes from the anus during defecation or on exerting force and remains protruded until forced back into the anus digitally ( with fingers).
Fourth degree hemorrhoids: The hemorrhoidal mass (Piles) remains protruded (out of the anus) at all times and can not be forced into the anal canal with the help of the fingers.
TREATMENTS
1.CONSERVATIVE TREATMENT: Oral medicines like kalyanaka Lehya, Kankayana Lehya, Manibadra Lehya, Sooranadi Lehya, Trivrith Lehya, Abhayarishta, Chirubilwadi Lehya, Chirubilwadi Kashaya And Kaidaryadi Kashaya are very usefull in piles according to the conditions. Laxatives, stool softeners, local anti inflammatory creams and warm water sits bath can help to reduce the symptoms. However second, third and fourth degree piles can not be cured with just oral treatment.
2. KSHARA KARMA: Kshara karma is a non surgical procedure of Ayurveda used to effectively in hemorrhoids. A alkaly medicine which is derived from a combination of various herbs is applied to the pile mass with the help of a special slit proctoscope. It is a type of chemical cauterization. This method is highly effective and can be performed on OPD and IPD level of Ayurdhama hospital.
KSHARA KARMA LIGATION: It is a simple non surgical procedure where a medicated thread is tied around the base of the pile mass and the piles mass will fall off within ten days. PREVENTION
To prevent hemorrhoids or Hemorrhoidal flare-ups:
1.Eat high-fiber foods
Fiber (roughage) is the part of plant food that is not digested. It stays in your gut and is passed in the stools (faeces). Fiber adds bulk to the stools.
Why is fibre important?
Stools (sometimes called faeces or motions) are usually soft and easy to pass if you eat enough fibre food and drink enough fluid. A diet with plenty of fibre will help to:
> Prevent constipation.
> Prevent diverticulosis (a common bowel condition).
> Prevent hemorrhoids (piles) and anal fissure (a painful condition of the anus).
> Reduce weight.
> Possibly reduce the risk of developing bowel cancer.
High fiber foods include the following:
> Whole meal or whole-wheat bread and biscuits.
> Whole meal flour used for baking and cooking.
> Fruit. vegetables and nuts.
2. Drink plenty of liquids
How much water and other fluids should you drink daily? The average man – who bums about 2,900 calories daily – needs 2,900 ml, or about 12 cups, of water each day. The average woman – who bums 2,200 calories daily- needs about 2,200 ml, or about 9 cups, of water each day.
3. Exercise:
Stay active to reduce pressure on veins which can occur with long period of standing or sitting and to help prevent constipation. Exercise can also help you loose excess weight.
4. Don’t Strain
Straining and holding your breath when trying to pass a stool creates greater pressure in the veins in lower rectum.
Go as soon as you feel the urge. If you wait to pass a bowel movement and the urge goes away, you stool could become dry and be harder to pass.
Self – Care
You can temporarily reliew the mild pain, swelling and inflammation of most Hemorrhoidal can be controlled with the following self-care measures:
- Apply an over-the-counter hemorrhoid cream or suppository containing hyrocortisone, or use pads containing witch hazel or a topical numbing agent.
- Keep the anal area clean. Bathe (preferably) or shower daily to cleanse the skin around your anus gently with warm water. Soap isn’t necessary and may aggravate the problem. Gently drying the area with a hair dryer after bathing can minimize moisture, which can cause irritation.
- Soak in a warm bath several times daily.
- Apply ice packs or cold compresses on the anus for 10 minute~ up to four times a day.
- If a hemorrhoid has prolapsed, gently push the hemorrhoid back into the anal canal.
- Use a sitzbath with warm water. You can get one at a medical supply store or some pharmacies.
- Use moist towel or wet toilet paper after a bowel movement instead of dry toilet paper.
- These self-care measures may relieve the symptoms, but they won’t make the hemorrhoid disappear.
- See your doctor if you don’t get relief in a few days.

WHAT IS AN ANAL FISTULA?
Anal fistula, or fistula-in-ano, is an abnormal connection or channel like structure, between the surface of the anal canal and the exterior perianal skin.
Anal fistulae originate from the anal glands, which are located between the two layers of the anal sphincters and which drain into the anal canal. If the outlet of these glands becomes blocked, an abscess can form which can eventually point to the skin surface. The tract formed by this process is the fistula.
CAUSES OF FISTULA?
Anal fistulas commonly occur due to an anal abscess.
An abscess is a collection of pus and infected fluid. An anal abscess usually develops after a small gland, just inside the anus, becomes infected with bacteria.
A fistula may occur if an abscess has not completely healed, or if the infected fluid has not been entirely drained away.
An anal fistula may also develop as a result of
> a growth or ulcer (painful sore)
> a complication from surgery
> a congenital abnormality (a health problem that you were born with)
Anal fistulae are also a common complication of conditions that result in inflammation of the intestines. Some of these conditions include:
> Irritable bowel syndrome (l8S): a chronic (long-term) disorder that affects the digestive system, causing abdominal pain, diarrhoea and constipation.
> Diverticulitis: the formation of small pouches that stick out of the side of the large intestine (colon), which become infected and inflamed.
> Ulcerative colitis: a chronic condition that causes the colon to become inflamed and can cause ulcers to form on the lining of the colon.
> Crohn’s disease: a chronic condition that causes inflammation of the lining of the digestive system.
SYMPTOMS OF FISTULA
Anal fistulae can present with many different symptoms such as:
> Pain
> Discharge – either bloody or purulent
> Pruritus ani- itching around the anus
> Systemic symptoms if abscess becomes infected
DIAGNOSIS OF FISTULA
Diagnosis is by examination, as an outpatient setting or under local anaesthesia. The examination can be done using proctoscope.
Possible findings:
> The opening of the fistula onto the skin may be seen
> The area may be painful on examination
> There may be redness
> A discharge may be seen
> It may be possible to explore the fistula using a fistula probe (a narrow instrument) and in this way it may be possible to find both opening.. of the fistula

KSHARA SUTRA :
Kshara Sutra is a seton thread medicated with organic alkalis. The alkali is repeatedly coated on the seton thread 15 – 21 times. Apart from this, natural antibiotic like haridra powder, guggulu, etc are also used to make Ksharasutra.
The mechanical action of the threads and the chemical action of the drugs coated, collectively do the work of cutting, curetting, draining, and cleaning the fistulous track, thus promoting healing of the track/ wound. This also acts both as the antiseptic and fibrotic agent to induce healing.
The process of healing starts from deeper tissues and moves towards the periphery. This can be applied and changed periodically till the thread cuts the fistulous tract. Since the sphincter heals by fibrosis, there is no incontinence.
Under local anaesthesia, the kshara sutra is inserted into the tract and the two ends of the thread are tied forming a loop. The alkalis coated on the thread are continuously released throughout the length of the track there by cutting, curetting, draining cleansing and healing the track. This therapeutic action of the thread lasts for seven days.
The old thread is then replaced with a new thread following the same procedure. Depending on the length of the tract and the extent of damage, the kshara sutra may be changed up to 5 times. The changing of the thread is a simple procedure taking about 1 to 2 minutes and requires no anesthesia.
BENEFITS OF KSHARA SUTRA
> The procedure does not require hospitalization for more than 4 to 5 hours
> The patient requires minimal bed rest and can resume daily activities within 12 – 24 hours
> No painful dressings required
> The drugs coated on the Kshar-Sutra are slowly and gradually released into the track and the wound, leaving no abscess overseen. These abscess are drained out by the action of the drugs.
> The sphincter muscles are not dissected and hence the possibility of incontinence is ruled out.
COMPLICATIONS OF FISTULA
Depending on the effectiveness of the treatment, fistula may result in infection or incontinence of stools.
RECURRENCE OF FISTULA
The re occurrence rate of fistula treated with kshara sutra ligation procedure is less than 2%. This is because the medicines on the thread gradually and continually curate the payogenic membrane and fibrous tissue in the track and thus leave no pus pockets un drained.

Prevention Of Anal Fissures
For fissures in adults:
> Keep the anorectal area dry
> Wipe the area with soft materials, a moistened cloth, or cotton pad; avoid rough and scented toilet paper
> Promptly treat all occurrences of constipation and diarrhea
> Avoid irritating the rectum
Complications Of Anal Fissures
> Anal fissure that fails to heal. An anal fissure that doesn’t heal can become chronic, meaning it lasts for more than six weeks.
> Anal fissure that recurs. If you’ve experienced anal fissure once, you have an increased risk of another anal fissure.
> A tear that extends to surrounding muscles. An anal fissure may extend into the ring of muscle that holds your anus closed (internal anal sphincter). This makes it more difficult for your anal fissure to heal. An unhealed fissure may trigger a cycle of discomfort that may require medications or surgery to reduce the pain and repair or remove the fissure.
